Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.  There is further information about countries and vaccinations required on the links below

Europe Europe & RussiaNorth America North America
Central America Central Americaindia South America
egypt Carribeanlithuania Africa
bulgaria Middle Eastpoland Central Asia
china East Asiachina Australasia and Pacific

It is important to make this initial appointment as early as possible  - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations.  These vaccines have to be ordered as they are not a stock vaccine.  Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

We do not offer URGENT or IMMEDIATE travel services as this tends to place an unreasonable pressure on our nursing staff. Should you require urgent travel services then please contact www.privategpservices.co.uk Tel 01245 234134 or www.nomadtravel.co.uk Tel 01279 653694. We are happy to offer you travel services at the surgery if you give us reasonable notice.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.

Travel Health Questionnaire

To help us offer the appropriate advice, please fill out the following online form before coming to see the nurse:

Travel advice and form

General Travel Health Advice

You can then either: return the form to the reception, e-mail it to freshwell.administration@nhs.net or fax it to 01371 811282. This will give the nurse the information required to check the latest requirements. The nurse will then contact you to make an appropriate appointment. If you are emailing this form back to us, please do fill it in but do not change the format of the document as we are currently unable to read ".docx" documents.
