October 2017

Our children's flu vaccines have now arrived. Children who were 2 or 3 on 31st August 2017 are eligible. Please ring for an appointment if you would like your child vaccinated. Primary school aged children will be vaccinated at school.

We have now installed hearing loops at both the reception and dispensary counters. We would like to thank The CREAMER Fund for financing this important improvement to our practice for patients who are hard of hearing. We have added signs to each counter to help them know where to stand for the best reception. There is also a portable device available for use in the consultation rooms. Please ask at reception if you would like this to be made available when seeing a doctor or nurse. the portable device can be used without a hearing aid if necessary.

September 2017

We are now taking bookings for our flu clinic on Saturday 7th October. Eligible ADULT patients (over 65s, pregnant women, carers, patients with certain chronic diseases, immunosuppressed and obese patients) should phone reception to book. Further information relating to flu vaccinations for children will follow shortly.

Building works are currently underway at the surgery. Because we are having a new reception counter fitted the main entrance is not in use. Please follow the signs to enter at the rear of the building. We apologise for any inconvenience this may cause.

Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to complete the application form below in order to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

To help us offer the appropriate advice, please fill out our travel advise form and return it to the surgery before booking an appointment with our Practice Nurse Team. 

Please remember we need a MINIMUM of 6 week notice before your departure date to assess your travel form, advise you of your vaccination needs and arrange appointments if appropriate.  Failure to give appropriate notice will mean we are unable to offer you this service and you will need to make alternative arrangements with another Travel Clinic.

Providing travel advice and treatments are not covered under our NHS contract.  Fees will be applicable for any treatment given. 

Travel advice and form

You can then either: return the form to the reception, e-mail it to freshwell.administration@nhs.net or fax it to 01371 811282. This will give the nurse the information required to check the latest requirements. The nurse will then contact you to make an appropriate appointment. If you are emailing this form back to us, please do fill it in but do not change the format of the document as we are currently unable to read ".docx" documents.
