Advice

If you are severely immunosuppressed patients and aged 16 years old and above) you are now invited to book a 4th (booster) dose of Covid vaccination if you have had three doses already.

The Fourth (Booster) dose can only be done as a walk in. Local information on walk in clinics can be found here.

The Third dose can be booked online at or as s a walk in as above or by calling 119 or our Referral Support Service on 0300 123 0771

We have sent eligible patients a text message advising them that they need a 3rd dose or at 4th (Booster) dose. Further information can be found here.

Patients who are having difficulty accessing their third or fourth dose can ring Referral Support Service on 0300 123 0771 Mon-Fri 8am to 6pm and they will support patients.

If you cannot book appointments online, you can call 119 free of charge. You can speak to a translator if you need to.

If you have difficulties communicating or hearing, or are a British Sign Language (BSL) user, you can use textphone 18001 119 or the NHS 119 BSL interpreter service.

People over the age of 12 who believe they may be eligible for a 3rd primary dose of the vaccine because they have a weakened immune system can now get it with or without a referral letter from their clinician on presentation of relevant medical evidence and clinical assessment.

Medical evidence confirming eligibility for a third primary dose includes:

  • A letter from your GP or specialist confirming eligibility and timing of a third primary dose.
  • A hospital letter that describes the person's condition at the time of the first and/or second dose
  • Evidence of prescribed medication at the time of the first or second dose, either in a hospital letter that describes the medication being prescribed, a prescription copy or a medication box with the patient’s name and a date on it.

COVID Pass

This practice is not able to provide you with a letter that shows your COVID-19 vaccination status. Please do not contact the practice about your COVID-19 vaccination status unless you have been advised to by the 119 service.

People in England who have been fully vaccinated against COVID-19 can use the NHS COVID Pass to demonstrate their COVID-19 status.

If you’re aged 18 or over, you can get an NHS COVID Pass for domestic use where certain venues and events in England choose to use COVID-19 certification.

If you’re aged 12 or over you can get an NHS COVID Pass for travel abroad but you should follow the entry requirements of the country you are travelling to. See the GOV.UK website for more information about the NHS COVID Pass for children.

A full course is currently two doses of the Moderna, AstraZeneca or Pfizer vaccine, or one dose of the Janssen single-dose vaccine. Vaccine status will be available through the NHS COVID Pass service from:

  • the NHS App (ages 13 and over) which you can download from app stores
  • the NHS website (ages 12 and over)
  • calling 119 and requesting a paper letter

You will need to register to use the online services, if you have not already. It may take more than a week for your identity to be checked and verified.

If you cannot access the online services, and you have had both doses of the vaccine, you can request a paper letter from the NHS by calling 119. Only call 119 if you are due to travel outside the UK in the near future and have had your second dose (or one dose of the Janssen single-dose vaccine) more than 5 working days ago. It may take up to 7 working days for the letter to arrive

When you're planning your travel, you should check the latest information on demonstrating your COVID-19 status when travelling abroad on the gov.uk website. Make sure there is enough time to get proof of your COVID-19 vaccination status before you are due to travel.
