If you aren't able to complete the form online, you can print off the forms below or collect from Reception. Every person needs to fill in both PRF1 form and the additional Freshwell Questionnaire

What happens next

Online registrations will be completed within two days. Please allow up to one week for paper registration forms to be processed. Once your registration is complete to will get a confirmation text from us.

 

You electronic medical notes will be sent to us electronically and they normally arrive within a few days. Your paper medical notes will be sent over from your previous practice, but this may take several weeks.

 

If you are taking regular medication, you should book an appointment as soon as possible for a medication review. We cannot prescribe any medications for you until you have had an appointment. Our dispensary will take 5 days to prepare your prescription for collection

Secondary use of GP data Opt-Out

The NHS uses information about patients (patient data) to research, plan and improve:

  • the services we offer
  • the treatment and care patients receive

They get this data from your GP surgery, hospitals and other healthcare providers. The organisation that collects your data is called NHS England.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for between 14 days and 3 months. 

This will allow you to be on the local practice list and still remain a patient of your permanent GP. 

To register as a temporary patient come in to reception to request a temporary registration form.
