A wellbeing initiative from your Primary Care Network


What this initiative is about

Thriving in Later Life is a new PCN-wide initiative for people aged 75+ focused on:

·         Positive conversations about staying well, active and confident

·         Understanding what helps people thrive as they get older

  • Identifying anyone who may benefit from extra support
  • Bringing people together to share ideas and experiences

It combines proactive phone calls, community activities, and local co-design of healthy ageing support.

Proactive wellbeing calls

Our health coach will be making friendly, 10–15 minute calls to people aged 75+ across the PCN.

Each call will:

  • Explore what helps people stay active, confident and connected
  • Offer information about local groups and activities and frailty prevention (if wanted)
  • Use a simple supported self-assessment to record a Clinical Frailty Scale (CFS) score
  • Identify anyone who may benefit from follow-up from the PCN Frailty team or social prescribing team

New community activities for 2026

As part of this initiative, we are starting:

• A “Thrivers” Parkrun/Walking Group

A friendly weekly walking group linked to parkrun - open to all fitness levels. A social, no-pressure way to stay active and connected.

• A Later-Life Wellbeing Discussion Group

A space for people to share experiences, offer tips, and shape what “thriving in later life” should look like in our area.  This will launch based on interest gathered during the calls.
